Disability Services

Working with people with a disability and the people who support them has always been an important part of what we do. The NDIS has changed the way people with a disability access their care and support. You set your goals – we help you meet them.

Whether you want to get out and about in your community, get back to the workforce or develop your communication skills, we’re here to help. When you tell us what your goals are, we look at how we can best help you meet them. No two participants are the same, which is why we always work with you to make sure you are getting the most out of your NDIS plan.

The NDIS and you

The NDIS is the National Disability Insurance Scheme. The NDIS gives you the freedom to choose who supports you, and the flexibility to work with a range of service providers to help you reach your goals.

You might be eligible for the NDIS if you:

  • live in Australia and are an Australian citizen or permanent resident
  • are under 65 years old
  • have a lifelong impairment or disability that stops you from doing everyday things by yourself

You can check your eligibility for the NDIS here. When you’re ready, you can apply to the National Disability Insurance Agency (NDIA) and they will get you started.

How we work is changing

The NDIS is a huge step forward for people with a disability. It's also a big change for service providers. We believe that the best way to support you is to collaborate with the other members of your support team.

Aged Care

We’ve been supporting older Australians for more than 13 years. We provide support in your home, in hospitals and in aged care facilities. It’s our job to find the right person to come and work with you. And it’s a job we take very seriously.

We believe that living longer at home means enjoying the life you've built.

Consumer Directed Care

We offer all of our services using a Consumer Directed Care (CDC) model. This means that we work with you to ensure you have:

  • true choice and flexibility in how you receive care, and who provides it
  • all of the information you need to make an informed decision about the care that suits you best
  • a clear understanding that we work in partnership with you to help you reach your goals
  • transparency around your plan, your care team and the costs of your care

Consumer Directed Care puts you squarely at the centre of every decision we make with you.

Home Care Packages

Recent changes to Home Care packages have made it easier for you to have the freedom and flexibility to choose who comes into your home to help.

We don't charge exit fees if you decide to move to another Home Care Package Provider. This is just one of the ways we support you in exercising your right to independence.

Veterans’ Home Care

We’re very proud of the work we do with our veteran community. The Veterans’ Home Care (VHC) program is designed to provide basic, practical support to veterans who need some help in their own home. It doesn’t offer support for complex care needs.

Veterans' Home Care includes services like:

  • domestic assistance, including cleaning your house, shopping, paying your bills and doing the ironing
  • personal care, which is help with self-care for up to 1.5 hours per week
  • safety-related home and garden maintenance, including minor maintenance and repair work
  • respite care, to give your carer a temporary break from caring duties

Most of the services you can access through the Veterans' Home Care program attract a small co-payment fee that you are responsible for.

If you think you are eligible for support through the Veterans’ Home Care program, your first step is to get in touch with the VHC Assessment Team on 1300 550 450. Once they have done their assessment, we're here and ready to help.

Home Care Packages

If you live in Perth, Adelaide or Melbourne and have a Home Care Package, we'd love to talk to you about how we can support you. Whether your Home Care Package is level 1, 2, 3 or 4, we have the right staff ready to deliver the best care.

What is a Home Care Package?

Home Care Packages are funded by the Commonwealth Government. A Home Care Package can fund services like home and garden maintenance, transport, personal care, a helping hand with meal preparation, nursing care and social support. They are a great way for you to access support that will help you stay in your own home, longer.

Recent changes mean it's now much easier for you to choose your Home Care Package provider. You have the flexibility and freedom to change providers if you are looking for a better fit.

How to apply for a Home Care Package

If you think you are eligible for a Home Care Package, the first step is to get in touch with your local Aged Care Assessment Team. The easiest way to do this is by visiting the My Aged Care website. The ACAT assessment is free, and once it’s done, the ACAT team will let you know what types of supports you need to safely and happily stay in your own home.

Four levels of support for you

There are four levels of Home Care Packages:

  • Level 1, for people with basic care needs
  • Level 2, for people with low care needs
  • Level 3, for people with intermediate care needs
  • Level 4, for people with high care needs

Services you can access

Your Home Care Package helps you access support while still in your home. Our team are ready to support you with:

  • keeping your home clean and tidy
  • doing the heavy lifting, weeding, and mowing in your garden
  • taking you shopping, or doing your shopping for you
  • preparing nutritious meals with you - or for you, if you prefer!
  • taking you to appointments or social engagements
  • social support, which can be sharing time together over a cup of tea, or going with you to social functions
  • personal care to make sure you can shower and dress yourself safely and independently
  • nursing care, which can include wound care, continence management and medication management

Options to increase your Home Care Package funding

In some cases, the Commonwealth Government can offer you additional funding to your Home Care Package. Each supplement usually adds 10% of the total value of your package to your available funds.

  • Veterans' Supplement
  • Dementia and Cognition Supplement
  • Oxygen Supplement
  • Viability Supplement
  • Enteral Feeding Supplement

It's important to note that if you are eligible for both the Veterans' Supplement and the Dementia and Cognition Supplement, you will usually only receive the Veterans' Supplement.
